- Connecticut residents only
- Currently attending dental school and in good academic standing

Applicant's must submit the following 3 items:
- Scholarship Application
- Financial Aid Officer Form
- Letter of Recommendation from Dean of Students or equivalent.

Please submit all items by October 15, 2023 to be considered for this year's scholarship.
Scholarship recipients will be notified by December 31, 2023.

The amount awarded for the HWC scholarship is a minimum of $1,000. Recipients will be chosen based on their financial needs, essay questions and recommendation letter. The number of scholarships awarded each year will vary based on available funds and number of qualified applicatants.

Online Applications will be submitted directly to the Horace Wells Club. Please be sure to download the Financial portion of the application below and submit to your school's Financial Officer.

In addition to the application and financial aid form, letters of recommendation from the Dean of Students (or equivalent) of your dental school can be emailed to info@HoraceWellsClub.com or mailed to:
